Description:
It is not uncommon for hospice patients and their families
to travel, taking them out of a hospice's service area for
a temporary period of time. It is important that both the
HOME hospice and the TRAVEL hospice understand their respective
responsibilities to assure a safe and well coordinated trip.
This program provides valuable information for all involved
in the process: both hospices, the patient and their families.
Objectives:
- Participant will list the regulatory issues which affect
contracting for traveling Hospice Medicare Benefit (HMB)
patients
- Participant will discuss responsibilities of the Home
Hospice, the Travel Hospice and the patient/ patient representative
- Participant will identify important areas of agreement
between the Home & Travel Hospice
