Description:
This program provides a historical, as well as current overview
of documentation opportunities for improvement. The presentation
includes a case study to demonstrate how to incorporate multiple
diagnoses into determining clinical eligibility.
Objectives:
- At the end of this session, the learner will be able to
describe the progress made in documenting clinical eligibility
since November 2007.
- At the end of this session, the learner will be able to
understand the process used to determine and document a
patient's eligibility for hospice care.
- At the end of this session, the learner will be able to
develop and document an individualized care plan and visit
notes that reflect the patient's terminal condition and
ongoing decline.
