Description:
These two sessions from Weatherbee's Hospice Regulatory
Boot Camp in Denver, Colorado focus on perhaps the most challenging
new condition of participation: Initial and Comprehensive
Assessment of the Patient.
Part 1 - Presents the CoP and its five standards with a review
of time frames and strategies for implementation.
Part 2 - Reviews tools and processes, common disease trajectories
and types of IDG assessment documentation that support hospice
eligibility.
Objectives:
- Learner will list the five standards that comprise this
condition of participation
- Learner will describe the time frames for completion of
the initial assessment, comprehensive assessment, and assessment
updates.
- Learner will describe the tools and processes needed to
comply with this COP and its standards
- Learner will list the three end stage trajectories commonly
associated with a hospice diagnosis.
- Learner will describe the types of IDG documentation that
will support hospice eligibility.
