This two-part presentation from Weatherbee's Hospice Regulatory Boot Camp in Denver, CO. focuses on challenges and implementation strategies for the new IDG, Care Planning and Coordination of Services CoP.
Part 1 - Introduces and reviews the CoP and its standards.
Part 2 - Discusses implementation of the CoP and its standards, with an emphasis on  the team, the tools, and the techniques needed to meet the requirements.

Objectives:

• Participant will name the five standards that comprise this Condition of Participation.
  
• Participant will state time frames required for plan of care review.
  
• Participant will describe connection between plan of care and patient assessment.
  
• Participant will describe hospice's responsibility for coordinating with other health care providers to fill any gaps in the patient's overall comprehensive plan of care.
  
• Participant will describe an effective  IDG meeting format.
  
• Participant will list the functions of the IDG meeting facilitator, timekeeper, and scribe.
